A shemale is a transwoman, someone who was designated male at birth (born with a male body), and who works in the adult entertainment industry. Most transwomen have estrogen therapy (take hormone for feminisation purposes) and possibly have some gender-related surgery, like facial feminisation or breast augmentation. Their male genitalia is intact, i.e., has a penis, scrotum, etc. The only way to have "naturally" functional male genitalia is to be born with a male body. However, while taking estrogen, it is difficult to achieve and maintain an erection; it is common for transwomen in the porn industry to use stimulants (think Viagra). Or they may quit hormones to regain male sexual function, or they may get all changes with surgery and never take hormones.

Please don't use the word "shemale," it is offensive and dehumanizing, reducing a person to a fetish and sex object. Instead, use "transwoman" or "MTF (male-to-female) transgender." Use MtF transsexual when referring to people who will eventually have genital surgery.
