You have to use the gale boomerang. Throw it at the monster when he is "Looking" at you. Then throw it at him. Once you do that, go over to his and hit him with your sword. ;)
You need to use the hammer. It depends which part of the temple on the isle of ruins you are talking about. You don't have to hit on the tiles themselves, you can hit the edges and it will flip the tile next to it. Whichever tile you hit, the tiles surrounding it will flip.
What you do is: Start on the green tile (top left from your perspective). Go all the way down to the bottom left (in a straight line), then go diagonally to the top right corner. Go left again to the right tile, then move diagonally down to the bottom-right tile (the red tile). If this explanation is a bit confusing, just watch the warp phantom on the right side of the room. Mimic the path he's walking.

At the bottom of the screen there should be a cracked tile .Touch the tile and then wait till u see a face.tap on the face and the mouse should come out.

Tile worms can only be defeated with the use of the Gale Boomerang, the temple item. If you don't have it yet, there is nothing you can do but run away. Once you obtain it, you can Z/L-target them and they will be ripped out of the ground. Quickly slash at them while they are vunerable or they will burrow back into the ground.

In legend of Zelda, the floor is made up of different squares. The red and blue patterns are made of squares. Using the hammer to hit the ground will make the square you hit and all the ones around it change state. Hitting somewhere that isn't a tile will still change the ones around it.
