The entire dash assembly needs to be removed to gain access to the blend door actuator. Big job, suggest acquiring a shop manual of you plan on doing this job yourself.
Take old one out put new one in
Unfortunally you need to remove the entire dash assembly in order to gain access the the blend actuator. Consider replacing the heater core while your in there only because the amount of work involved to do it later if it should leak.

The temp blend door actuator is probably at fault.
It's a temp blend door and actuator, and it is located behind the dash assembly, connected to the HVAC box. The dash assembly needs to come out to access the blend door actuator.
The heater blend door is not run by vacuum. It is run by an electric actuator.
The temp blend door actuator is broken.
