Because he has shown signs of self-mutilation, the best thing to do is call help for him, or talk to his parents. You should not put yourself in a position in his life because it'll show him the wrong signs (may wanting him back) and would make it worse. You need to talk to someone about this and get him help because once he gets in a deeper state of this then you're going to have problems getting him out of it.
