This is a very very simple contact form which you can make as complicated as you need to from here. All it asks for is a name, email address and some comments.
You'll need to create 2 files - feedback.html and process.php.
feedback.html
process.php
$name = $_POST['name'];
$email = $_POST['email'];
$headers = "From: ".$email;
$comments = $_POST['comments'];
$you = "typeyour@emailhere.com";
mail($you, "Feedback recieved from ".$name, $comments, $headers);
Create an HTML form with METHOD="post" and ACTION="login2.php". In login2.php check if the entered username and password are correct. You can get the username with $_POST['name_of_username_input']. The same goes for the password.

Any program capable of editing a text file can be used to create a form in PHP. For those that have a strong understanding of PHP, a simple text editor, such as Microsoft Windows Notepad is sufficient for creating forms in PHP. For less experienced PHP developers, they should consider using a program such as SciTE or Eclipse in order to give them syntax highlighting and code correction features (features vary by application). Regardless, you can expect to have to write the majority of the form handling routine by hand, unless you use a PHP library or script that you find and download from the Internet. Some developers have already written some generic form-handling scripts-- if a PHP developer can use their functionality, it can save countless hours of manual coding, and instead just "link" into the library (using the "include" or "require" statements), and call the appropriate functions.
