The Gladiator-type titles (Gladiator, Merciless Gladiator, Triumphant Gladiator, and so on) can be gained by being among the top 5% of arena players on your server. This comes with the title and a mount. These can only be earned in their respective seasons.
Second, there are the classic titles (Grunt/Private) which can no longer be earned, but will be re-used in Mists of Pandaria for pvp-related achievements. They used to be awarded by simple PvP honor gained in the days of classic WoW.
Finally, there are some titles that are awarded by PvP achievements, like Battlemaster (for doing basically all possible achievements in all Battlegrounds) or Of the Alliance/Horde (for killing all racial leaders).

Some achievements will give you titles. As far as PvP titles go, they were stopped in one of the expansions. According to blizzard pvp titles will make a return in Cataclysm.

Yes, on a PvP server everyone is on PvP by default and can be attacked by the opposing faction. I believe the only exception is the starting areas for characters below level 10, and the Sanctuaries (Shattrath and Dalaran).

This would be a purely personal preference between PVP and PVE as to which is better. There are pro's and con's to each, and only you can decide which is better.
