Lots of practice. There is no actual aiming technique associated with holding a weapon that way. It stems from Hollywood producers, who would have actors hold their guns sideways in order for a good view of both the gun and the actor to be presented on film, but it was the movie Juice which is where the transition from film to popular culture was made.

This actually inspired a spoof advertisement, for a product supposedly called "Homeboy Nyte Sytes" (see related link at the bottom).
