i would draw a picture of myself, then draw an arrow to body parts and write a brief thing about each part of me: for example:

Print out a picture of yourself big enough to fit on said index card, apply paste (or other adhesive substance) to the backside of the printed picture, apply the pasted picture to the index card, and when the paste dries, fold the index card over the picture and tape/staple shut.
