Just replace the ballcock valve..
It's not the toilet, there's a blockage in the pipes some where after the drain pipe from the toilet joins up with the drain pipe from the tub.You're going to need to have the pipe snaked.
take the lever off completely and see if something is preventing it seating properly. I fix many Moen faucets and have never heard of this propblem.

That depends on what is wrong with it.
Get a new toilet
Disconnect the chain from the lever,and using a pair of pliers turn the nut on the lever counter clockwise. These threads are reverse from the norm.
