Most of getting a cool (fluke) throwing knife in MW2 is luck. But at the start of the game if you know the enemies spawns you will have a larger chance. I, myself, have got 2 random throwing knives at the start of the match and 3 as Game Winning Kills. For the GWK throwing knives, keep an eye on the score. Try and spawn trap the enemy and then when the score reaches 7300 or 7400 throw a knife into their spawn. Hope I helped.
