I presume you mean on the PC as the X-BOX and PS3 versions are lacking, but you require a "premium" Steam account.
Steam is the program you launch Team Fortress 2 inside,
If you have bought 0 games on Steam then you can only add a certain amount of people as friends, to do this you would click ALT+TAB (default) and then click "View Players" then click "Profile" next to someones name and click "Add Friend" on their profile page.
If you haven't bought anything on Steam then your account will remain as a free account and can only add so many people, this cannot be changed.
