Contact your county MH/MR agency. Sometimes, agencies dealing with disabilities will point you in the right direction.

Sadly, when nobody with money is willing to step in, the disabled person is permanently homeless, although for the disabled, that may not be long. Being poor and disabled doesn't result in long life.
