If you're referring to the quest given at Tenpenny Tower, the Ghoul underground is located in Warrenton metro station, to the southwest of the Tower. Roy Phillips is located there, along with some other ghouls.
If you're having trouble finding the station: the first time you discovered Tenpenny Tower, you'll remember that Roy Phillips was yelling into the intercom, and left after a short while. Walk in the general direction he was going, and you'll come to the metro station.

One of the only ways is too find dogmeat (something along those lines) in the scrapyard and then find a human/ghoul/supermutant follower.
Alot of times that has to do with the reactors shutting down, raids, exposure to radiation and such. It is annoying sometimes. There is a vault i found, i believe it was fallout 3, where there were a bunch of clones of one guy. They were all hostile and I never discovered anything about the place.
Occasionally Fallout: New Vegas will freeze, but not as frequently as Fallout 3.
