How do you give items to the medicine man on Shark Tooth Island?

You simply just click on him and give an ingriedient to him, but you need to have all the ingriedients. Make sure you put on a grass skirt (from Booga Bay vendor) so the medicine man will talk to you.

If it is not already in your items, you may have to reset the island to continue. The bone should be down in the pit next to the statue with the crawly worm. First, get the key ingredient and the coconut milk (Main Street), get and wear the grass skirt from Booga Bay, and climb the vine to see the medicine man. If he takes the ingredients, then it was just a glitch on the Item Bag. If not, click the small map and "Restart Island." (see also the related question)

You have to go to the crazy man, on top of the tree. Collect all the items he tells you, then go back to the top of the tree and he will give you a special coconut that you shoot at the shark.
Where do you go after you get the bone on shark tooth island?
Continue left and get the Key Ingredient and leave the underground ruins. The bone, Key Ingredient, and coconut milk go to the Medicine Man in his bamboo "tree house" that somehow drops coconuts. First get a grass skirt to wear (so you fit in), then push the block over and climb the vine to have him mix your three items into a sleeping potion. Take the coconut to the cannon and fire it into the water, and soon the shark will be sleeping with the fishes.
