There are two ways to know if pads are worn. One is if i u hear a squeaking noise. Two is to look through the rim at pads and if u don't see any padding then need to change them. If u cant tell by looking through the rim then u will have to take the tire and caliper off to get to pads. If pad is more then half way gone or uneven then change them. If pads are uneven also replace the caliper.

It depends on the present condition of the break pads, and how worn they are. If your break pads need replacing I recommend having them replaced as soon as possible.

When you're brake pads be calm worn, they will have a slight shimmy when you apply the brakes. When the brake pads are completely worn out they will make a grinding noise.

Couple things come to mind. -improper caliper operation -abnormally worn brake pads -rotor plate abnormally worn -even a suspension missalignment
Air in the brake lines. Severly worn break pads. Worn brake boost piston. Cracked brake line on that brake. Loose brake line.
Its probably the metal wear indicator on your brake pads squealing to let you know that your pads are almost worn out.
you probably need a new set of break pads. they have most likely worn down to the metal so that'd be why its grinding
Check your car handbook about the brake lights which should light up on the dashboard if the brake pads are worn. If the brakes are noisy and screeching it may suggest worn out pads. You will have to jack up the car and take the wheels off to inspect the brake pads to see if they are worn out.

Assuming that the brake pads were just changed and the sensor is still on, Perhaps the sensor wire was broken during installation causing the sensor to stay on. Happened on my 2001 a4.
