Well, either your temperatures are inconsistent, often melting things and refreezing them; the other possibility is that if you have a fridge with a conjoined water dispenser, then that is leaking and pooling at the bottom to freeze.

Dispenser
In the case of automatic soap dispenser, to open it you should look underneath it. Behind the push bar you will see a small lever. Push this lever whereas gently pulling the top of the dispenser toward you. The top should swing away. If it is ordinary soap dispenser, you have to unscrew the pomp.
About 3/4 the way up each side of the dispenser, there are two indentations. Push on them at the same time and the housing will pop open.
There is a flush, white, rectangular button at the bottom of the dispenser, behind the hinge. Push it up with the tips of your forefingers and the front of the dispenser will make a clicking sound as the latch is released and the dispenser front opens.
