1) your batteries dying 2) you have a battery pack and not a rechargable one, and as a result of this (having the battery pack) the pack will pop out if ther contoller either vibrates or is dropped or 3) you have to sync your controller again with your system. If non of this works, than sorry man but you need a new controller
You do not have to use the PlayStation 3 wireless controller as a wired controller via the USB cable. Many people prefer the wireless option so they can freely move. The only advantage to use the charging cable is to provide a charge whem th battery is low the controller works with bluetooth and does not need to be pointed at anything to work
No it is no problem and a recommended method to charge the controller. I also put mine into my Sony BR6 HDTV USB port to change the controller because it is always on unlike the PS3 USB port that shuts off when the PS3 does.

it depends if it uses bluetooth or cord. if its a bluetooth u cant use it if lost if its cord just press the middle round circle button to activate it. that's how u activate cord controller.(u don't always have to plug it in its wireless if its cord)
