In the UK 6 inches down from the top of the door and 9 inches up from the bottom! this is because of the optical illusion created by the eyesight of 'Foreshortening' when looking down at the bottom hinge. In the USA the space is 5" and 10" with the exception of some western states where 7" and 11" is the norm. The middle hinge is always placed equally between the two.

this is OK if the door is flat and not decorative other wise you should line up the top of the hinge knuckle with the lower part of the top rail and the opposite for the bottom (IE measure the height of the top rail and that is the top hinge spacing and the height of the lower rail is the lower hinge spacing this purely for aesthetics if you are putting a 3rd hinge then either in between the two hinges or in line with the center of any center rails in the door). this is how i was trained when doing my apprenticeship as cabinet maker.
Fitting any sort of door is a difficult process requiring a high level of carpentry skills.

for an ordinary interior door and most exteriors also there's usually a small hole at the bottom of the hinge. Simply insert a nail or something similar up into the hole and tap with a hammer. Should pop right up, then use your fingers if it's really loose to lift it out of the hinge. If still tight use a screwdriver and tap it up and out. Another way is to tuck your foot under the door to support it and use a screwgun or screwdriver to remove the screws from the door portion of the hinges. Always start at the bottom and work your way up so your hands can control the door when the last top screw comes out. If the doors are really heavy, use shims to support the weight of the door and not your foot (ouch!). Helps to have two guys! If the hinges wont pop up they're probably frozen with rust or dirt. put some penetrating oil on them and let them sit overnight. Usually that'l do it.
Is the door a new door. Is the opening it's going into an existing opening with the hindges already cut in. You have to give more detail as to existing conditions.
It depends on the size of your door. You might just measure another door in your house and use those measurements.
Usually doors come with the hinge areas cut out. Interior doors use three hinges. Definitely want to match the hinges to the door jam.

Put your back to the jam where the hinges are. hold out your left arm. That is the direction the left hand door will swing.
No door hinges and cabinet hinges are two different things. Door hinges are generally much larger than cabinet hinges. Cabinet hinges and door hinges are not at all the same. They are different sizes and can withstand different amounts of weight.

A familiar example of what hinges do, are the hinges that are attached to a door.
Replace the hinges or attempt to bend them back by wedging a piece of wood under the door for a weekend. Replacing the hinges is your best bet.

spray some white lithium grease on the hinges and on the door stop. If that doesn't work put some weather strip around the edge of the door where the door meets the body.
put hinges on opposite side and do the same with the doorjamb.
