All you have to do is go to sounds in the media center and select "record new" and then record your sound!
go to: settings>controls panel>sound>windows sounds. all preset sounds are stored there. check what sound is assigned for each function. you can change those sounds with other files that you'd need to record.

Yes, you do need to plug a HD PVR into a computer to record.
I believe it's a hardware issue because it's only present when I record my audio. It sounds like a buzzing sound and like I'm in a cave.
