Open the tailgate and there is a bolt under the floor mat on the right
side, that you can attach your lug nut wrench on. Turn the bolt,
left using the lug wrench (lefty loosy, righty tighty) and it will lower
the spare tire on a cable from underneath the vehicle. Remove
the tire from the cable, and mount it on vehicle. Don't forget to
wind the cable back up before driving off.

The spare tire release bolt is located in the rear cargo area of the 1998 Toyota Sienna. Look for a small "U" shaped piece of loose carpet on the floor of the cargo area. The bolt should be turned counterclockwise to release the spare tire.
In Sienna 2006, spare tire is below the 2nd right seat.

under the car in the rear
There is no space for a spare tire if you have AWD. That is why factory provides the Run Flat Tires.
It is located underneath the vehicle in front of the right rear tire.
Yes, of course, it can fit in trunk.
If you have the Limited (aka XLE Limited) trim level then it was factory equipped with run-flat tires and there is no spare tire.
Not on the underside. A full size spare would be too close to the ground to be practical.
A spare tire on a 2002 Ford Ranger can be found on the underside of the rear of the truck. The tire can be removed with a ratchet.
