If you look very carefully at the base of the handle, it has a little wire clip that locks the handle onto the splined shaft by engaging in a groove in the shaft. I actually did this very thing yesterday on a friends car. What you need is a some steel wire, about 1.5mm in diameter (1/16") for the Americans out there, and use a pair of pliers to bend a small hook in the end of it. Bend it through about 100 - 110degrees. What needs to be done is the clip needs to be pulled out with this hook. With the end of the hook pointing outwards from the door try to hook the clip, and pull it out. You may need to try this from several different angles, as the clip may be facing one way or the other. When you have hooked it, give a sharp tug and it should pop right on out. Watch where it goes though, as it could travel quite a distance. Good luck!
The window winder is secured by a spring wire clip. Push the circular pad away from the winder handle and pull out the clip with a crochette hook or similar.

Begin by removing the inside door panel. Remove the linkage from the window winder. Remove the window winder retaining bolts. The window winder will come off.
Between the window winder and door card there should be a plastic ring about 8mm wide. Push this ring away from the end of the window winder. The windes should now slide off

There is a really thin horseshoe shaped circlip behind the winder handle . Use a screwdriver to gently lever the winder away from the panel and then use a small thin screwdriver to try and unhook the circlip . A bit awkward but trust me it will come off .

The winder fits onto a splined shaft, with a groove cut into it near the end. The winder handle has a spring clip, like an external circlip, which protrudes into the door card. The handle is simply pushed onto the shaft until the spring clip drops into the groove. To get it off, you need to find a way of pushing the spring clip off the winder handle. Try depressing the door card around the winder and look in to see if you can push the spring clip off, from around the handle shaft, using a screwdriver.
Just pull it!
With the handle end in the down position. Slide the spacer up. You'll hear a click. Then pull away the whole handle. (Note: The spacer is the whole plastic collar between the handle and the door card)
