Muth manufacturing supplies replacement mirrors (the glass part...no mechanicals) if the reflective backing is starting to separate from the glass. It sounds like yours may have broken guide pins or other internal problems. You still might want to check with them. If they can't supply the whole assembly (sans housing), they might be able to point you in the right direction.

Remove the plastic cover on the inside behind the mirror and then remove the three nuts holding ther mirror on. When you pull out the mirror unclip the wiring harness.
Try a glass shop
You have to take the door panel off on the inside of the door, and there are two bolts that hold the mirror housing. Its a real pain in the neck. If your just trying to take the mirror out of the housing and replace it, sorry I can't help you there.
Remove the front door sail panel. There you will see 3 bolts holding the side view mirror. Remove bolts, replace / or fix side view mirror. Installation is the reverse of removal.

To replace the passenger side mirror on the 1998 Nissan Maxima SE, first remove the door panel inside the door by releasing the clips below the panel and by unscrewing the handle screws. Then, remove the trim panel that covers the mirror mount inside the vehicle. There should be at least 4 screws. Release the wires from the old mirror and take the part off of the car. Add the new mirror and connect the wires. Replace the screws and the door panel.
There should be one torx screw on the bottom side of the mirror bracket, remove it and slide the mirror up, if the whole mirror has fallen off go to a parts store and they will have a kit to glue the windshield mount back on, then slide the mirror back on.
