Depends on the type. Some can't be replaced, you have to change the whole sash, frame and glass. Some are held in with strips that pop into place. If it is a wood frame, some have to be taken apart at the corners to get the glass out.

Several years ago I broke one pane of our Anderson double pane window in the garage. I decided to replace it but found out that the two panes are actually one piece of glass- its moulded into one piece. The biggest problem for insulated windows is that the seal breaks letting the gas escape, reducing the effectiveness of the window. This seal can break with damage to the frame.
The double pane window should be more efficient in keeping heat out. However, there is also more heat transfer from the frames if they are metal rather than vinyl.
