Cut the bolt head off with a cutting wheel then use a drill to drill out the bolt.

An accessory or idler pulley is frozen or in very bad shape and is snapping the tensioner and bolt from all the torque being applied to it. Check all your pulleys when you have it off. They should rotate freely.
It depends on which pulley. Locate the problem one and replace it. It may be the water pump or alternater in which case those components need replacement. Also may be the idler pulley is bad.
You need to take the belt off, and spin all the pulleys by hand and you should not be able to hear them turning. If you can then replace the one's that you hear because the bearings are bad in them. If they are all good then replace the belt. But I'm sure you will find a bad pulley somewhere.
I'm assuming you mean a pulley, which is what the belts run on. If so, depending on which pulley the bearings could be bad or if it is the crankshaft pulley (big pulley near the bottom) the rubber could be getting bad.
