When those things on my 92 B250 did not work, I replaced the speed sensor located on the transmission tailshaft and that fixed the problems. Hope that helped! There is no fuse for the fuel pump just a fuel pump relay.
If you open the odometer, there are two small plastic gears. One is connected to the speedometer, the other to the odometer. The speedometer gear drives the odometer gear. In all likelihood, one of these gears is missing a few teeth. I have the same problem with my Ramcharger, same year.

I recently changed the E.G.R. valve on my 1990 Dodge Spirit 2.5L (non-turbo). Now it is quiet @ idle & starts right up. I also notice an improvement in the gas mileage. Good Luck.
