The blower is below the dash on the passenger side. Remove the cover (two screws) and lower it to reveal the blower assembly. There are three bolts holding the blower on, and are a bit difficult to get at, especially the back one. You will need an extension bar for your socket set. The blower motor has a locking connector that is also a little tight to get off, but after that it drops right out. Replacement is reverse of this process.

Either it's the switch on the dash (unlikely) or the blower motor resistor pack (very likely). The resistor pack is usually easy to replace.
