The first thing you need to do is to remove the lens. As mentioned by others, slide a thin blade or plastic under the outside edge of the ledge and pop it off the two posts. The bulb looks as if it is a custom item, but is really an adapter.
Remove the burned-out bulb, and find a suitable replacement, for instance a #921. Straighten out the contacts, and carefully insert it into the adapter until it pops in. Using a needle-nosed pliers, slip each contact into its corresponding slit on the adapter. Then re-insert the assembly into the socket in the map light. Re-install the lens, making sure it pops over both posts.

The light will reset itself when the cause of the code has been repaired.

The map light on a 2004 Dodge Grand Caravan SXT is changed gently prying the edge of the cover to reveal the bulb. The bulb can then be popped out and a new bulb installed.
