This change out should be fairly easy (compared to the front rotors). I changed out the rear rotor on a 2000 Sebring Convertible rather quickly. Buy the rotor at Autozone or o'Rielly and you will see that it justs slips over the lug nuts.
The old one may take a little tapping around the edges to loosen it up. Use a gear puller in a worst case scenario.

when you remove the caliper on the disk brake by removing the bolts on the inside of the caliper the rotor just slides off, the only thing holding the rotor in place is the wheel.
They could if worn or warped to the safe and legal limit.
Have the tires Balanced. Then check alignment. If still shakes is it when your press the brakes? If so check the rotors and abs system. Or if it could also be the over drive if it is when shifting gears. Try running it with the over drive off.
If you hear the squealing when applying the brakes, the pads are worn out and need replacing ASAP. Failure to replace the pads immediately will result in worn out rotors which will also need replacing. If the squealing noise is not when braking it may be a worn drive belt.
you may need to replace your rotors and brake pads. to check this remove your tires and examine the rotors/pads for excessive wear.
