You've answered your question, already: you show. You act accordingly. Only yourself (and your ex) know whatever it was that hurt, that was unacceptable, therefore know what to do in order to convince him you will not slide down again. If you have indeed changed and want to conquer his trust back, you have a long, hard way ahead of you. It may take months before he notices the difference and before he gives you another chance.
