Assuming you are dealing with one of the newer (90+) mazdas, there are two vertical plastic pieces on the sides of the faceplate. Pop those off with a tiny screwdriver. Then you will need either a radio removal tool, or two narrow but long (~6") screwdrivers to insert into the two holes in order to release the clips holding the radio in. I believe that you have to push the tips of the screwdrivers towards the radio, but I can't remember for sure. Pull gently and wiggle the screwdrivers, and the radio should work itself loose.

The 89 is an s5 and the 87 is an s4 It will fit but might take minor modification.

You need to get the unlock CD that came with the deck. I have this same problem as the Jazz CD that came with mine was stolen in a CD case with my other stuff. If you can prove you bought the deck or it's registered to you, Eclipse will send you a replacement disc to unlock it. Good luck
Take it to a dealer, if you try to take it out without proper tools, the current cd/radio will disable itself and the radio deck may too with newer cars. Just stay on the safe side and call a dealer.
Buy a new deck at Best Buy ($80 or more) they will take your old one out and install your new one in for free.
