You don't. Replacement can cost $400 at dealer. Instead I use a NEODYMIUM CYLINDER MAGNETS 1inch diameter by 1inch thinck. The thickness is more important than the diameter so that it can hold together the visor and the roof. Magnet goes between the visor and the roof - you can't see it when the visor is in the up position.
Remove the sun visor retaining screws. The sun visor will slide off of the sun visor rod. Lubricate the sun visor rod. Put the sun visor back on.

I don't really know how to tighten it, its a tough job so i hear. ($400 or so) I have heard a couple of remedys for it, put a peice of velcro on the ceiling and visor, tie a peice of string from visor to a hook or something you put in your ceiling, or use something and just jimmy it in there so there is no room around the pole itself.
