You need to know both the Passport email and password of the Live account and the Passport email and password for the parent account associated with the Live account.
You can also do it easier online at xboxlive.com - just sign in with the Passport email and password of the Live account, go to "Manage Profile" from your home page, hit "Family Settings", sign in parent account, and you can change all the parental controls on the next page. From looking at that page, you basically want all options on the first button for each thing to turn off all restrictions completely.
To check it, just turn on your xbox. If working fine, its not damaged. If it doesnt turn on, you can either get it fixed by Microsoft or just use your controller to turn on/off your console.

Not if you are at the dashboard, this is with the new update that gives you avatars, go to the My Xbox panel. Scroll right to System Settings, click on Console Setting, go to Shut Down, Turn background downloads on. Now you can shut down your 360 and still be downloading. Once your downloads are complete then the Xbox will turn itself off.
Theres a little button on the 360 and the controller. Make sure you are holding both buttons when turning on the controller.
