You can unlock the map "Five" by completing the campaign on any difficulty or you can put in a code:
While in the main menu press LT, RT, LT, RT and you will stand up.
Go to the back of the room and press 'x' to use the computer.
Type in '3arc unlock' and then go to the Zombies menu and you will have 'Five' and 'Dead Ops Arcade'
