Quilts are used in the same way blankets and bedspreads are used on beds. They are laid over the top sheet, so that one sleeps between the sheets, rather than directly under a quilt. This protects the more fragile quilt from wear and tear. A fancy or "best" quilt also can be used as a decorative bedspread and removed from the bed when turning down the covers before sleeping.
