A provisional diagnosis is a temporary diagnosis based on initial assessments and may change as more information becomes available. A postoperative diagnosis is made after a surgical procedure based on findings during the operation and histopathological examination of tissue samples.
Being a provisional patient, you have a diagnosis that is not 100 percent certain and confirmed. The diagnosis is based on the information available to your health professional.

A provisional diagnosis is a temporary or tentative diagnosis made by a healthcare professional based on initial assessment and available information. It serves as a starting point for further investigation and may be confirmed, refined, or changed as more information becomes available through additional tests and evaluations.
A primary diagnosis is assigned when the presenting problems meet diagnostic criteria. A provisional diagnosis is assigned when the presenting problems meet some of the criteria for a disorder, but more information is required for a diagnosis to be assigned. An individual can be assigned both primary and provisional diagnoses depending on their presentation. For example, the diagnosis of Major Depressive Disorder; Cocaine Induced Psychotic Disorder, Onset during Intoxication (provisional), suggests that the client presents with depression and meets the criteria for MDD. In addition, the client appear to be experiencing psychosis as a result of consuming cocaine, however until the effects of cocaine wear off, the clinician does not know if the psychosis is a result of the cocaine, or is present when the substance is not in the body (which then may indicate a psychotic disorder). Therefore a provisional diagnosis is assigned.
it is provisional or probable diagnosis which is given by the physician in outpatient clinic and to confirm the diagnosis he advices u for different tests.
postoperative post-operative
Diagnosis code 998 is for other complications of procedures, not elsewhere classified. It excludes fluid overload due to blood transfusion, or TACO (transfusion associated circulatory overload). Code 998.0 is for postoperative shock which happened either during or resulting from a surgical procedure, but it excludes shock: anaphylactic due to serum; anesthetic; electric; following abortion; obstetric or traumatic. Code 998.00 is used for postoperative shock, unspecified, such as collapse, not otherwise specified, during or resulting from a surgical procedure or failure of peripheral circulation, postoperative. Diagnosis code 998 isn't a complete code...it must have 4 digits for more specificity. You can have 998.0 all the way to 998.9, but for most of those categories, you must choose a 5th digit. For example under category 998 I chose 998.5 which is for Postoperative Infection. Under that title, I chose 998.59 which is for "other postoperative infection".
It is not hyphenated.
