A radiator to heat a room functions by having hot water pumped through it. This heats up the metal and the air around the radiator heats up and slowly spreads around the room, heatong it up. The radiator in a car has the same principal but the opposite affect. Cold water is stored/pumped through the radiator and the hot engine heats the water up where it then goes elsewhere to cool off again. A large majority of modern cars are cooled more and more by air running over the engine directly but water is more efficient (it's easier to run air over the engine if you're driving than it is to get more cold water)
