It varies. In the US, individual states set the requirements. Normally there must be at least a certain number of inhabitants within the boundaries set forth in the municipal constitution (or charter, or whatever they call it in that jurisdiction) and the place must choose to call itself a city. In some places it also has to have a certain form of government, or one chosen from a short list, as well.
