In a sense, yes. :) the transition is. Just being transgendered in general has nothing to do with cosmetics, but a person's transition and surgery are very cosmetic.
Of course it is fine. It's something that a person is, and so there's no question of it not being okay. It's not a choice. And if you need to medically/socially transition, that's perfectly fine as well. Body/social dissonance is a legitimate reason for transition. There's no need to transition - someone who knows herself to be a woman is a woman, regardless of genitalia - but it is perfectly fine if that is something a person needs to do.
