It looks for signs of suspicious activity. Jagex has analyzed several existing bots, and claims that it can detect all of them.

Jagex does not publish details, because if it did, this would obviously make it easier for the bot-writers to bypass the controls. However, I assume that it looks for certain patterns in the behavior of bots. For example, to go from one place to another, a bot might walk on a certain very specific route, that is not typical for a player; or it might have a certain regularity in specific actions - real players will have a more random delay when repeteadly clicking, for example. These are just assumptions; as mentioned, Jagex does not publish details.

Note that using bots is not only against the rules; players who use them expose themselves to a very high risk, namely of losing their accounts. Jagex has a zero-tolerance policy with bots; if it finds a player using a bot - even a high-level player who took years to build up an account! - it will ban the account permanently.
