Normally they do not, unless the headlight is not working as a result of an accident. If it is just that the bulb has worn out on its own and needs to be changed, an auto insurance company would NOT pay for that. Fortunately it is not expensive to take care of on your own.

I'm guessing that you have clouded over or broken headlights and need to repair or replace them. If memory serves, the covers themselves are not removable. I think you need to replace the entire headlight assembly. There are kits available to resurface the plastic on the headlights, but they're expensive and there are varying reports as to how well they actually work.
