early 1900's families usually had a local family doctor, services were either paid in cash at that time or bartering was done, I live in a rural area and still barter with my family doc. we own a grocery store so he doesnt pay for groceries and if I need minor services he provides no charge. and I supply him with cases of paper towels and toilet paper for his office. When a poor family needed help he went out to the home delivered the baby, and was fed a meal or given a ham out of the smoke house. later when the wife was moving about, she would take him jars of canned good for his pantry or bake goods for the good ole doc and his wife. Consider my grandma has an appendectomy in 1923 and it cost 500.00 Dollars so payments were made to the hospital whom employeed a doctor on staff to provide such services. no one got rich, but everyone was happy at the end of the day.
