The scope of the appraisal should be focus on the items listed in their specific job description. As well, you could/should include items such as how they perform with respect to the companies cultural expectations or expected behaviors, their strengths, their areas for improvement, and a section in which you set mutually agreed upon goals for them to achieve by their next appraisal.
