Generally speaking the health of your eyes reflect the health of the rest of you. The lack of vitamin A will damage eyes. It is said you are more likely to use glasses if you read a lot, or even watch tv or use a computer a lot.

Yes you five year old should, because if he/she doesn't, his/her eyes will not be healthy when they grow up.
If you can see clearly, then chances are your eyes are healthy. You can look at your eyes in the mirror and check for discoloration, cloudiness, twitching, redness, or other abnormalities. Of course your doctor can tell you definitively if you have an issue with your eyes or if they are healthy.
