The nominal Head of State, the President, is chosen democratically by direct and secret ballot. However, candidates are vetted by Iran's religious leadership before they are allowed to even stand for Presidency. No Governmental or Presidential decision that is in the least bit controversial can be implemented in Iran without the approval of its supreme religious leader.
