In most states, a driver's license is valid for 3-4 years. They may expire on the anniversary of the day you got it, or they may expire on your birthday (California, for example, does this... no matter when you get your license, it expires on your third birthday after you got it, so your first license is valid for four years if you get it on your birthday, or three-and-a-bit if you get it on some other date).
There's no state that I'm aware of that makes you get a new license automatically after turning 18 (as opposed to getting one when the license expires).
However, it's become more common for states to place warning labels on licenses issued to young drivers so that if they're used as ID to buy cigarettes or alcohol the clerk can easily see the warning.
Because of this, if you've just turned 18 and are eager to get started on your lung cancer, you might want to get a new license without the UNDER 18 label. However, your old license is fine, and unless the clerk is horrifically stupid they should regard the label as just a warning to check the date closely and allow you to buy cigarettes if you're actually over 18 (you may need to point out this fact, though).
