You must first get married. Keep your spouse happy for a season or more and a scene will happen where you find out your wife is pregnant. 60 days later and the baby will be born. No matter how long you play he will never get any larger then just a toddler.

Yes. But only if you have a DS with a GBA slot and a copy of the game Friends of Mineral Town or More Friends of Mineral Town. Place your Mineral Town game in your GBA slot and play Harvest Moon DS like normal. Karen will visit every Tuesday and on certain festivals. Since she isn't in town very often it takes a long time to get her to a Red Heart. But you can marry her. However, the game ends if you marry any of the girls from Mineral Town.

Then... maybe months after that, the baby'll be born. I don't think you need to do anything special to get her pregnant. Just keep talking to her and giving her gifts that she likes. At least that's what I did, and my wife got pregnant. I'm not so sure when she'll get pregnant. Months after your marriage, perhaps? It's been a long time since I last played that game.
