This really depends on how many piggies you have and whether its in winter/summer etc.

in the summer i usually do it once a week or up to 10 days as they are only in it at nighttime. in the winter however, i do it at least once a week depending on how busy i am normally. If you can really start to smell them you obviously need to clean them out but be aware that male piggies tend to smell more than females anyway ( i have 2 males). i hope this helps you out a bit :D

It take about 30-40 hours to clean the cage. It may take longer if your guinea pig was murdered by masked ninjas.

i clean outmine 1nce a week but it all depends how big the cage is indoor cage 2-3 times a week outdoor cage 1 times a week

Well it probably depends on how many guinea pigs you have. So try cleaning it out at least once and week. If you can make twice, or three times at the most, even better.

When you clean out the cage you have to refill the hay, water, and food as well as change the bedding and scrub or dump out the bottom of the cage. So it should take around an hour.

while you can keep them out for a loooong time as long as they have food and water. but beware lol if you are holding them then they will pee on you usually if held for more then a half hour tops (at least my guinea pig lol). i almost always hold her with a towel because of this reason. without food or water i would say that you shouldn't keep them out for more then a couple hours (and yes if they are grazing grass counts as food and you can keep them out for at least up to 3 hours in this case). I'm not an expert so i would be careful. and remember if it is hot you should not leave your guinea pig outside unless it has a cool shaded place to go if it wants to and you should still not leave it out for more then half hour i would say if it is hot.
