Phil Lesh from the Grateful Dead first started playing the violin. In high school he switched to trumpet. He later learned to play the bass guitar when he joins the Warlocks in 1964.
The Warlocks was the original name of the Grateful Dead. Not 100% positive if they used it when they weren't welcome. Another Answer: Warlocks was indeed the original name of the band, (although there are differing theories and rumor of this), they did not however use the Warlock nor any other name to play in towns they were not welcome in. They just didn't play those towns.

On July 9, one month to the day before the Death of Jerry Garcia, the Grateful Dead played their last show at Soldier Field in Chicago, IL. According to Dead Base, an organization that has been tracking set lists for the Dead since the early 80's this was their 2,318th show.
The Dead did not play any gigs in November 1986. They PLayed in July and did not play again until December.
