This is not mandated anywhere. Basically, just as many as the particular form the issuer purchases or makes has. The side that goes to the various governments is now done electronically, not by hard copy, and the option of electronic form is more available and increasingly used by people too.

You can always run it through a copyig machine if you need another copy.
