In the history of the shuttle program, no shuttles have ever crashed.
However, two Orbiters were lost due to catastrophic malfunctions.
On January 28, 1986, 73 seconds after the launch of its 10th mission, the Challenger, STS 51-L exploded killing all seven crew members.
On February 1, 2003 near the end of its 28th mission, the Columbia, STS-107 disintegrated upon re entry, killing all seven crew members.

None, the space shuttle had not been designed until after we quit going to the moon.
None crashed they broke apart and there were only two Space Shuttles that did so. Space Shuttle Challenger broke apart on take-off over the Atlantic Ocean in 1986 and Space Shuttle Columbia broke apart on re-entry over East Texas in 2003.
Space shuttle Endeavour has not crashed. The shuttles Challenger and Columbia disintegrated in January 1986 and February 2003 respectively.
Currently NASA have three space shuttles Discovery, Atlantis and Endeavour.
No new space shuttles are being built at this time.
